To schedule or cancel an appointment or to inquire about any specific information please call or email us. We are open Monday through Friday, with late nights on Tuesday and Wednesday until 7pm.

New Patients

New to Byron Family Dental? Welcome, it is a pleasure to meet you. Here is what to expect before, during, and after your first appointment so there are no surprises.

Before Your Appointment

Forms to complete

  • New Patient Form
  • Transfer of Records Form (for your previous dental office records)
  • Insurance information, if applicable
When You Arrive

What to bring

  • A list of current medications
  • An accepted form of payment
  • Your insurance ID and plan number, if applicable
During Your Visit

First-visit services

  • Dental and medical history review
  • Dental health assessment
  • Gum health assessment
  • Oral cancer screening
  • X-rays if necessary
  • Treatment diagnosis and discussion
  • Time for any patient concerns or questions

After your initial exam, patients receive an individualized checkup and cleaning schedule. Free estimates are available for any additional diagnosed treatment.

Payment Info

Please note payment is expected at the time of service. We file most insurance claims as a courtesy for you at no cost. Most dental insurances are intended to cover some, but not all of the cost of your dental care, so any uncovered balance is the patient's responsibility.

Office Policies

We do our best to run on time, and these policies help us keep the schedule fair for every patient.

Late Policy

Arriving more than 15 minutes late

If you arrive more than 15 minutes past your scheduled appointment time, we reserve the right to cancel the appointment and reschedule for an alternate date.

Cancellation Policy

Two business days notice

Two full business days notice is required for standard appointments. Four business days notice for half-day or full-day appointments.

Short-notice cancellations may be subject to a fee.

Insurance Policy

We file claims for you

We file most insurance claims as a courtesy for you at no cost. Most dental insurances are intended to cover some, but not all, of the cost of your dental care.
